lsa: cleanup operator<<

Delete .toString() virtual function in favor of ostream output.

Delete Lsa::getOriginRouterCopy() in favor of a custom Boost.Multi-Index
key extractor.

Mark single-parameter constructor as 'explicit'.

Rewrite class-level Doxygen with ABNF syntax.
